What You Will Do:

• Carry out assessments to identify the needs of children with disabilities
• Develop and implement tailored care and support plans
• Work collaboratively with multidisciplinary teams and partner agencies
• Monitor and review interventions to ensure effective outcomes
• Advocate for children and young people, ensuring their voices are central to decision-making
• Provide guidance and support to families and carers

Requirements:

• Qualified Social Worker with Social Work England registration
• Experience working with children with disabilities or within children's services
• Strong assessment, safeguarding, and care planning skills
• Ability to work both independently and within a team
