What you will do:

  • Collaborate closely with families and children with disabilities to ensure their needs are met efficiently and empathetically.

  • Conduct assessments, develop care plans, and monitor progress to provide tailored support for each child under your care.

  • Work with a multi-disciplinary team, maintaining strong communication and coordination to deliver cohesive support services.

  • Participate in case conferences and court proceedings, preparing detailed reports as required.

  • Handle a caseload primarily involving children in care, child protection, and children with disabilities, using your expertise to promote positive outcomes.

  • Coordinate with external agencies and professionals to ensure holistic support is provided to children and families.
