Key Responsibilities

• Carry out comprehensive assessments for children with disabilities, identifying individual needs and required support.

• Review and evaluate existing care packages to ensure they continue to meet the child and family's evolving circumstances.

• Work collaboratively with families, carers, and service providers to deliver effective and sustainable care solutions.

• Present informed recommendations to resource allocation panels regarding care planning and service provision.

• Engage with multidisciplinary professionals to support holistic decision-making and coordinated care.
