What you will do:

• Arrange and coordinate respite provision for children and families

• Undertake home visits to assess needs, promote safeguarding, and support child welfare

• Complete comprehensive assessments to identify the needs of children with disabilities and their families

• Advocate for children and families to ensure access to appropriate services and resources

• Develop, implement, and review care and support plans in partnership with multi-agency professionals

• Work closely with occupational therapists and other specialists to deliver holistic care

• Arrange and manage direct payments in line with local authority procedures

• Undertake risk assessments to promote safe and supportive environments

• Participate in looked after children reviews where applicable

• Complete parenting assessments to support family functioning

• Organise and attend core group meetings to coordinate multi-agency interventions
