What you will do:

• Conduct thorough assessments for children with disabilities and their families to identify support needs.

• Develop and implement personalised care plans that prioritise each child’s wellbeing.

• Work collaboratively with multidisciplinary teams, including health professionals, educators, and family members, to ensure holistic support.

• Provide guidance, advocacy, and ongoing support to families, helping them navigate available resources and services.

• Regularly review and update care plans to maintain their effectiveness and relevance.
