What You Will Do

  • Carry out statutory assessments under the Children Act 1989 for children with disabilities.

  • Develop detailed, outcomes-focused care plans with children, families, and multi-agency professionals.

  • Identify safeguarding concerns and work collaboratively to protect vulnerable children.

  • Provide specialist support to children with a range of disabilities, facilitating access to short breaks, therapies, and specialist services.

  • Support transition planning into adulthood, promoting independence and ensuring a smooth transfer to adult services.

  • Manage a varied and complex caseload, maintaining accurate case notes and completing all required reports.

  • Build strong working relationships with health, education, and voluntary sector partners to deliver holistic, person-centred support.
