What You Will Do

  • Provide professional casework and support to a caseload of older adults, ensuring their needs are met.

  • Conduct thorough assessments, develop personalised Pathway Plans, and complete Risk Assessments.

  • Work collaboratively within multi-disciplinary teams, liaising with agencies, organisations, and families to deliver positive outcomes.

  • Maintain accurate records and case reports in line with statutory requirements and organisational policies.

  • Attend and contribute professionally to meetings, conferences, and court hearings as required.

  • Promote diversity and implement Equal Opportunities practices within the team and wider organisation.

Qualifications and Experience

  • Recognised Social Work qualification and HCPC registration.

  • Evidence of continuing professional development, including Post Qualification Award (PQA) or equivalent.

  • Experience working within multi-disciplinary teams, particularly with older adults.

  • Proven ability to manage a demanding caseload and communicate effectively, both verbally and in writing.

  • Strong understanding of relevant legislation and confident application in practice.

Salary range

  • £37,938 - £43,693 per year
