What You Will Do

Provide professional casework and support to a caseload of older adults, ensuring their needs are met.

Conduct thorough assessments, develop personalised Pathway Plans, and complete Risk Assessments.

Work collaboratively within multi-disciplinary teams, liaising with agencies, organisations, and families to deliver positive outcomes.

Maintain accurate records and case reports in line with statutory requirements and organisational policies.

Attend and contribute professionally to meetings, conferences, and court hearings as required.

Promote diversity and implement Equal Opportunities practices within the team and wider organisation.
Qualifications and Experience

Recognised Social Work qualification and HCPC registration.

Evidence of continuing professional development, including Post Qualification Award (PQA) or equivalent.

Experience working within multi-disciplinary teams, particularly with older adults.

Proven ability to manage a demanding caseload and communicate effectively, both verbally and in writing.

Strong understanding of relevant legislation and confident application in practice.
