Key Responsibilities
1. Assessment & Intervention

Complete proportionate Care Act assessments, mental capacity assessments, support plans, and reviews.

Identify strengths, risks, and outcomes using a person-centred approach.

Apply Making Safeguarding Personal in all Section 42 safeguarding enquiries.
2. Case Management

Manage a varied caseload within statutory timeframes.

Develop, implement, and review personalised support plans.

Work collaboratively with adults, carers, families, and professionals.
