What you will do
• Assessment: Undertake assessments of adults' needs, strengths, and support requirements within the community.
• Care Planning: Develop, implement, and review person-centred support plans that promote independence and wellbeing.
• Case Management: Manage a varied caseload of adults requiring social care support and intervention.
• Multi-Agency Working: Work collaboratively with health professionals, community organisations, carers, and partner agencies.
• Community Visits: Conduct visits across Lewes and North Wealden, including rural locations where access to transport is essential.
• Advocacy: Support individuals to access services and resources that improve their quality of life and achieve positive outcomes.
