What you will do:
- Conduct comprehensive assessments of adults with learning disabilities, creating person-centred care plans tailored to individual needs.
- Collaborate with multi-disciplinary teams to implement and evaluate care plans, ensuring the support provided promotes independence and empowerment.
- Provide guidance and support to individuals, families, and caregivers, promoting strengths-based and evidence-informed practices.
- Actively contribute to safeguarding processes and ensure compliance with the Care Act and Mental Capacity Act.
- Manage complex caseloads efficiently, maintaining detailed records and reports on progress and outcomes.
- Engage in negotiation with various agencies and professionals to coordinate holistic support services.
Essential:
- Fully qualified Social Worker with current registration for instance with Social Work England.
- Proven experience in adult social care, with a particular focus on supporting those with learning disabilities and autism.
- Strong understanding and application of the Care Act, Mental Capacity Act, and safeguarding legislation.
- Exceptional skills in managing complex caseloads, underpinned by the ability to perform under pressure.
- Outstanding communication, negotiation, and interpersonal skills.
- A committed approach to person-centred, strengths-based, and evidence-informed practice.
