What you will do:

  • Conduct comprehensive assessments of adults with learning disabilities, creating person-centred care plans tailored to individual needs.
  • Collaborate with multi-disciplinary teams to implement and evaluate care plans, ensuring the support provided promotes independence and empowerment.
  • Provide guidance and support to individuals, families, and caregivers, promoting strengths-based and evidence-informed practices.
  • Actively contribute to safeguarding processes and ensure compliance with the Care Act and Mental Capacity Act.
  • Manage complex caseloads efficiently, maintaining detailed records and reports on progress and outcomes.
  • Engage in negotiation with various agencies and professionals to coordinate holistic support services.

Essential:

  • Fully qualified Social Worker with current registration for instance with Social Work England.
  • Proven experience in adult social care, with a particular focus on supporting those with learning disabilities and autism.
  • Strong understanding and application of the Care Act, Mental Capacity Act, and safeguarding legislation.
  • Exceptional skills in managing complex caseloads, underpinned by the ability to perform under pressure.
  • Outstanding communication, negotiation, and interpersonal skills.
  • A committed approach to person-centred, strengths-based, and evidence-informed practice.

Salary range

  • £32 per hour
