Key Responsibilities

• Community Response and Reablement Team:

  • Provide information, advice, and reablement services to new community members.

  • Assist individuals in maintaining their independence within their homes.

• Community Independence and Wellbeing Team:

  • Deliver essential social work and occupational therapy services to individuals across diverse care settings.

  • Promote community integration and support for vulnerable adults.

• Hospital Team Support:

  • Work with the hospital team to deliver comprehensive 7-day social work and care services, supporting individuals, carers, and families during the hospital discharge process.

  • Ensure smooth transitions from hospital to home, ensuring that individuals' social care needs are met.

• Collaboration with Community Teams:

  • Address the needs of individuals with learning disabilities and mental health challenges.

  • Coordinate with healthcare and social services professionals to provide comprehensive support to service users.

• Community Engagement:

  • Develop and implement strategies to build strong, supportive relationships with the local community.
