What you will do:

Prepare for social work involvement by reviewing case records and liaising with relevant professionals

Undertake assessments of need for service users, carers, and families, including complex and high-risk situations

Develop, implement, review, and update Care and Support Plans

Design and manage care packages to address social care, health, and wellbeing needs

Support individuals and families to make informed choices under Self-Directed Support and Individual Budgets

Apply safeguarding procedures and take appropriate action where abuse or neglect is suspected

Manage and assess risk, promoting positive behavioural change to reduce risk

Undertake crisis assessments and apply relevant legal interventions when required

Maintain accurate, confidential, and up-to-date case records

Participate in, and where appropriate chair, meetings and decision-making forums

Advocate for service users, carers, and families within organisational and legislative frameworks
