What you will do:

  • Undertake Care Act 2014 assessments, reviews, and reassessments, ensuring care and support plans are person-centred and promote wellbeing and independence

  • Identify, assess, and respond to safeguarding concerns, contributing to safeguarding plans and participating in multi-agency meetings

  • Conduct Mental Capacity Assessments, apply relevant legislation, and support individuals and families to understand their rights

  • Collaborate with health professionals, housing providers, and other partners to deliver coordinated support, attending multidisciplinary meetings and case discussions

  • Manage a caseload of adults with complex needs, ensuring commissioned services are of high quality and provide value for money

  • Maintain accurate and timely case records, participate in supervision and training, and contribute to service improvement initiatives

Person Specification:

  • Hold a degree in Social Work (BA / MA / DipSW) and be registered with Social Work England (SWE)

  • Experienced in Adult Social Care within a local authority, particularly in undertaking Care Act assessments and safeguarding

  • Desirable experience in working with adults with complex or long-term needs
