What you will do:

- Conduct thorough and Care Act compliant assessments and reviews of adults’ care and support needs, considering their physical, emotional, and social requirements.
- Use the guidelines set out in the Care Act to determine ongoing eligibility for services and support clients in 24-hour care scenarios.
- Apply the principles of the Mental Capacity Act and facilitate best interest decision-making processes whenever necessary.
- Collaborate with individuals, families, and other healthcare professionals to gather relevant information and develop comprehensive care plans.
- Advocate for individuals' rights and ensure timely receipt of appropriate support services.
- Maintain detailed, precise records and documentation in line with established procedures.
- Stay up-to-date with legislative changes, policies, and best practices relevant to social work and community support services.
