What you will do:

  • Provide day-to-day operational management of the Reablement Team, ensuring effective service delivery and performance management.

  • Lead, motivate, and support staff through supervision, appraisal, capability management, and workforce development.

  • Ensure reablement interventions are person-centred, outcome-focused, and time-limited.

  • Oversee case allocation and management, ensuring appropriate prioritisation and robust risk management.

  • Ensure compliance with Adult Social Care legislation, including the Care Act 2014, Mental Capacity Act 2005, safeguarding responsibilities, and local policies.

  • Monitor service performance, outcomes, quality standards, and KPIs, taking action to address any shortfalls.

  • Manage budgets and resources effectively, ensuring value for money and efficient service delivery.

  • Work collaboratively with health partners, hospitals, community services, and external agencies to support safe hospital discharge and prevent unnecessary admissions.

  • Lead on service improvement, transformation, and quality assurance initiatives.

  • Manage complaints, incidents, and escalations in line with organisational procedur
