What You Will Do:

• Carry out comprehensive assessments of individuals and families in need of support
• Develop and implement tailored care and support plans
• Work collaboratively with multidisciplinary and partner agencies
• Maintain accurate, timely, and compliant case records and reports
• Advocate for service users to ensure their needs, rights, and voices are central to decision-making
• Support effective service delivery and smooth transitions between care pathways

Requirements:

• Qualified Social Worker with Social Work England registration
• Experience in locality, adult, or children's social work (as applicable to service need)
• Strong assessment, safeguarding, and care planning skills
• Ability to manage complex caseloads at a senior level
