What you will do:
- Conduct comprehensive assessments of individuals’ social care needs within the community
- Develop, implement, and review personalised care plans in collaboration with other health professionals
- Provide expert guidance and support to service users and their families

Requirements of the Social Worker:
- Degree or equivalent in social work
- Current registration with Social Work England
- Substantial experience in frontline social services within the UK as a qualified Social Worker
