What you will do:

  • Provide specialist social work support and intervention to individuals and families experiencing homelessness.
  • Conduct comprehensive assessments to identify needs, risks, strengths, and appropriate support pathways.
  • Develop and implement effective case management plans tailored to individual circumstances.
  • Work collaboratively with local authorities, charities, housing providers, and other agencies to secure sustainable housing solutions.
  • Advocate for service users, ensuring they can access appropriate housing, health, social care, and community resources.
  • Manage complex cases and provide professional guidance to colleagues where required.
  • Maintain accurate case records and documentation in line with statutory and organisational requirements.
  • Identify and respond appropriately to safeguarding concerns.
  • Promote independence, resilience, and positive outcomes for individuals and families.
  • Contribute to multi-agency meetings and partnership working to improve service delivery.
