As a Senior Practitioner, you will bring your expertise and skills to our teams, holding a small caseload, and be supported to make evidence-based decisions. You will work as part of the locality management team to support performance and lead practice. Experience the diversity and challenges of working with customer groups with various health and social care needs, including mental health, learning disabilities, physical disabilities, frailty, and safeguarding. We will also support you to build and develop identified specialisms and maintain your professional development.
What you will do:
- Hold a small but complex caseload and lead on case reviews to ensure best outcomes.
- Supervise and support team members, fostering a culture of continuous improvement and professional growth.
- Implement evidenced-based decisions in accordance with the Care Act 2014.
- Engage with a wide range of stakeholders to develop community assets and enhance service provision.
- Conduct person-centred, asset-based, outcome-focused assessments, care management, and reviews.
As a Senior Practitioner, you will be:
- Registered with Social Work England.
- Holder of a Social Work qualification with 3 years of post-qualified experience in Adult Social Care.
- Skilled in line management and supervision.
- Proficient in person-centred, asset-based, and outcome-focused approaches to assessment and care management.
