Purpose of the Role

To deliver high-quality social work assessment, care planning, and intervention for adults being discharged from hospital or requiring rapid intervention. You will work within a multi-disciplinary RIACT team to ensure safe, coordinated, and timely transitions, supporting independence and reducing hospital readmissions.

Key Responsibilities

  • Conduct comprehensive social care assessments for adults needing hospital discharge or rapid support.

  • Develop person-centred care and support plans addressing needs, risks, and strengths.

  • Liaise with hospital staff, ward managers, and multidisciplinary teams to plan safe and timely discharges.

  • Facilitate smooth transitions to home, residential care, or community services.

  • Identify, manage, and escalate safeguarding concerns for vulnerable adults.

  • Collaborate with health colleagues, therapy services, voluntary organisations, and care providers.

  • Maintain professional registration with Social Work England and adhere to statutory frameworks.

Person Specification

  • Qualified Social Worker registered with Social Work England.
