What You Will Do:

• Complete Care Act assessments and Mental Capacity Act assessments in hospital settings.

• Fulfil statutory duties to support safe and timely discharges.

• Manage a varied and complex caseload within acute hospital environments.

• Work closely with multidisciplinary teams to coordinate effective care planning.

• Provide high-quality, person-centred social work interventions.

• Work both independently and collaboratively to deliver responsive services.

• Travel between sites to support regional hospital services where required.

Requirements:

• Qualified Social Worker with relevant adult services experience.

• Strong background in hospital or discharge-focused social work.

• Experience completing Care Act and Mental Capacity Act assessments.

• Ability to travel across sites - access to a car required.
