With a focus on intermediate care at Willowbank, this 12-month locum position offers the chance to make a tangible difference in the lives of your community members. You will be providing essential support to service users and their families, fostering independence and positive outcomes. This role involves collaborating closely with Manchester Local Care Organisation partners, managing and assessing risks, and developing individualised support plans. Ideal candidates should be available to work two days a week at Willowbank Care Home in South Manchester and one day a week at the Gorton hub in Central Manchester. You should also be available for weekend shifts, scheduled every one in seven weekends. Your role will involve contributing to Multidisciplinary Team discussions and supporting discharge and care planning. Shifts run from 8am to 3:30pm, 8:30am to 4pm on weekdays, and 9am to 4:30pm on weekends.
