What you will do:

• Conduct thorough assessments of individuals with learning disabilities under the Social Services and Well-being (Wales) Act 2014, ensuring person-centred outcomes.

• Manage a diverse caseload, including complex cases and safeguarding concerns, using your professional judgement to support vulnerable adults.

• Develop and regularly review care and support plans, promoting independence and community participation.

• Lead and participate in safeguarding enquiries, implementing effective risk management strategies.

Person Specification:

Qualifications:

• Degree in Social Work (BA/MA or equivalent)

• Evidence of continued professional development

Registration & Compliance:

• Registered with Social Care Wales

• Knowledge of relevant legislation including the Social Services and Well-being (Wales) Act 2014 and the Mental Capacity Act 2005

• Enhanced DBS clearance

• Familiarity with Safeguarding Adults procedures in Wales
