Key Responsibilities

As a Hospital Discharge Social Worker, you will:

  • Deliver a high-quality statutory social work service within the Adult Hospital Discharge Team.

  • Facilitate safe, timely, and effective hospital discharges in line with national discharge guidance.

  • Undertake Care Act 2014 assessments using a strengths-based and person-centred approach.

  • Coordinate and implement care packages, working closely with reablement services, home care providers, and community support services.

  • Complete Mental Capacity Assessments and make Best Interests decisions where required.

  • Undertake safeguarding enquiries and risk assessments in accordance with statutory duties.

  • Work collaboratively with NHS clinicians, discharge coordinators, occupational therapists, and community health professionals.

  • Communicate effectively with service users, families, carers, and advocates to support discharge planning.

Salary range

  • £32 per hour
