What You Will Do:

• Carry out Care Act 2014 assessments to identify and address service user needs
• Undertake Mental Capacity Assessments and support informed decision-making
• Make Best Interest decisions in collaboration with families and professionals
• Work effectively in a fast-paced environment delivering high-quality support
• Collaborate with the Short Term Support Service – Prevention team
• Engage with community resources to improve outcomes for service users
