We are seeking a skilled Adults Social Worker to join the Preparation for Adulthood Team for a rewarding six-month locum assignment. You will be based at Howden House, Union Street, Sheffield, and working part-time at your convenience within three to four days per week totalling 21 hours. What you will do:
- Conduct Care Act assessments for young individuals making the critical shift into adulthood ensuring they each have a tailored pathway to success.
- Engage in meticulous support planning and reviews to uphold top-quality standards for service users.
- Undertake essential Mental Capacity Act assessments and make Best Interests decisions safeguarding the well-being of clients.
- Facilitate and support Court of Protection applications when necessary demonstrating a tenacious attention to detail and compliance.
- Tackle complex casework collaborating closely with young individuals and families to provide a seamless transition.
- Strategise effective transition pathways ensuring that each young person's journey into adult services is smooth and positive.
Requirements:
- Hold a Social Work qualification with registration in Social Work England.
- A robust experience foundation within Adult Social Care specifically transition processes.
- Comprehensive knowledge of the Care Act, Mental Capacity Act, and the Court of Protection is essential.
- Demonstrated ability in managing intricate caseloads with professionalism.
- Provide three years' satisfactory references with a reliability check on employment gaps.
