What you will do
- Strength‑Based Assessments: Conduct assessments using a strengths-based approach to promote independence and wellbeing.
- Integrated Working: Collaborate with partner agencies to deliver coordinated, holistic care.
- Mental Capacity & Safeguarding: Complete Mental Capacity Assessments and Section 42 enquiries in line with the Care Act 2014.
- Hospital Discharge: Support safe and timely hospital discharges where required.
- Duty Work: Manage duty responsibilities and respond to urgent cases effectively.
- Case Recording: Maintain accurate, timely records and documentation in line with statutory standards.
Qualifications / Requirements
Essential
- Qualified Social Worker with current Social Work England (SWE) registration.
- Strong working knowledge of the Care Act 2014 and adult safeguarding frameworks.
- Experience completing Mental Capacity Assessments and Section 42 enquiries.
