What you will do
- Case Management: Manage a varied caseload of adults with complex mental health needs.
- Assessment: Complete and review Care Act assessments, ensuring person-centred and outcome-focused support planning.
- Mental Capacity: Undertake Mental Capacity Act assessments and make Best Interest decisions where appropriate.
- Safeguarding: Lead and contribute to safeguarding enquiries under Section 42 of the Care Act.
- Multi-Agency Working: Collaborate with NHS mental health services, community teams, and partner agencies to deliver integrated support.
- Care Planning: Develop, implement, and review effective care and support plans.
- Strengths-Based Practice: Promote independence, recovery, and resilience through a strengths-based approach.
- Professional Reporting: Prepare comprehensive assessments, reports, and court documentation where required.
- Record Keeping: Maintain accurate, timely, and compliant case records.
- Statutory Responsibilities: Deliver services in accordance with legislation, council policies, and professional standards.
