Role overview:
As an Adults Social Worker in the Long Term Planning Team, you will manage long-term and complex cases, delivering Care Act-compliant assessments, reviews, and support planning. You will work in partnership with individuals, carers, and multidisciplinary teams to promote independence, wellbeing, and safety.

What you will do:

Assessment & long-term support planning

  • Conduct Care Act assessments, reviews, and reassessments for adults with long-term needs

  • Develop and review person-centred care and support plans focused on independence and wellbeing

  • Manage long-term cases, ensuring support arrangements remain appropriate and effective

Safeguarding & risk management

  • Identify, assess, and respond promptly to adult safeguarding concerns

Salary range

  • £32 - £35 per hour
