About the Role

An exciting opportunity has arisen for an experienced Adults Social Worker to join the Learning Disabilities Team in South Tyneside. This ongoing locum position offers a competitive hourly rate of £26.93, with flexible part-time and full-time options available to support your work-life balance.

You will play a key role in supporting adults with learning disabilities and complex needs, promoting independence, wellbeing, and positive life outcomes through strength-based and person-centred practice.

Key Responsibilities

Assessment & Care Planning

  • Undertake comprehensive strengths-based assessments in line with the Care Act 2014.

  • Develop, review, and manage person-centred care and support plans for adults with learning disabilities.

Mental Capacity & Legal Duties

  • Complete Mental Capacity Assessments and make Best Interests decisions in accordance with the Mental Capacity Act 2005.

  • Prepare reports for the Court of Protection where required, ensuring full legislative compliance.

Safeguarding

  • Lead and participate in safeguarding enquiries under Section 42 of the Care Act.

  • Promote independence while effectively assessing and managing risk.

Multi-Agency Collaboration

  • Work collaboratively with health professionals, housing services, and other partner agencies to deliver coordinated care.

  • Support smooth transitions from Children's Services to Adult Services, enabling access to employment, education, and community resources.

Case Management

  • Manage a varied caseload, maintaining accurate and up-to-date records in line with statutory requirements and council policies.
