About the Role
An exciting opportunity has arisen for an experienced Adults Social Worker to join the Learning Disabilities Team in South Tyneside. This ongoing locum position offers a competitive hourly rate of £26.93, with flexible part-time and full-time options available to support your work-life balance.
You will play a key role in supporting adults with learning disabilities and complex needs, promoting independence, wellbeing, and positive life outcomes through strength-based and person-centred practice.
Key Responsibilities
Assessment & Care Planning

Undertake comprehensive strengths-based assessments in line with the Care Act 2014.

Develop, review, and manage person-centred care and support plans for adults with learning disabilities.
Mental Capacity & Legal Duties

Complete Mental Capacity Assessments and make Best Interests decisions in accordance with the Mental Capacity Act 2005.

Prepare reports for the Court of Protection where required, ensuring full legislative compliance.
Safeguarding

Lead and participate in safeguarding enquiries under Section 42 of the Care Act.

Promote independence while effectively assessing and managing risk.
Multi-Agency Collaboration

Work collaboratively with health professionals, housing services, and other partner agencies to deliver coordinated care.

Support smooth transitions from Children's Services to Adult Services, enabling access to employment, education, and community resources.
Case Management

Manage a varied caseload, maintaining accurate and up-to-date records in line with statutory requirements and council policies.
