What you will do

• Undertake Care Act assessments and reviews using a strengths-based approach.

• Develop and review person-centred care and support plans.

• Promote independence, wellbeing, and informed choice for adults with learning disabilities.

• Complete reassessments as individual needs change.

• Identify and support unpaid carers, completing Carer's Assessments where appropriate.

• Conduct Section 42 safeguarding enquiries under the Care Act 2014.

• Assess and manage risk in accordance with safeguarding principles and legislation.
