Key Responsibilities

• Carry out comprehensive assessments to identify patient needs and required support

• Develop and review care and discharge plans in line with best practice and legislation

• Work collaboratively with medical staff and partner agencies to deliver holistic care

• Support patients and families through hospital admission and discharge transitions

• Maintain accurate, timely and compliant case records and documentation

• Participate in multidisciplinary meetings and case reviews

• Advocate for patient rights and access to appropriate services

• Keep up to date with relevant policy, guidance and legislation in hospital social work
