You will play a vital role in ensuring individuals receive appropriate care and support following discharge, working within a fast-paced hospital environment and collaborating closely with multidisciplinary professionals.

• Conduct Care Act assessments for hospital inpatients and develop timely, effective discharge care packages.

• Complete Mental Capacity Assessments and contribute to Best Interest decisions where required.

• Manage safeguarding enquiries in line with statutory responsibilities.
