What you will do

• Deliver a professional social work service to adults, their families, and carers, adhering to the Care Act 2014.
• Complete accurate and timely statutory reviews with adult residents.
• Conduct strength-based assessments and create support plans that highlight personal, community, and universal resources.
• Employ robust, evidence-based decision-making to ensure best value and promote independence.
• Integrate services by collaborating with all relevant professionals around the individual.
• Develop creative, personalised solutions to empower individuals to manage their lives as independently as possible.
• Encourage the use of direct payments whenever feasible and perform ongoing assessments of service users, including risk assessments.
• Identify and address safeguarding and quality of care issues, referring them to the appropriate staff or agency to protect clients' welfare and optimise support quality.
