What you will do
- Case Management: Manage a varied caseload of adults with complex care and support needs.
- Assessment: Undertake strengths-based assessments in accordance with the Care Act 2014.
- Care Planning: Develop, implement, and review personalised care and support plans.
- Safeguarding: Complete safeguarding enquiries and contribute to safeguarding investigations and processes.
- Mental Capacity: Carry out Mental Capacity Act assessments and Best Interests decision-making where required.
- Multi-Agency Working: Collaborate with health professionals, community organisations, and partner agencies to achieve positive outcomes.
- Hospital Discharge: Support individuals through hospital discharge planning and ongoing care arrangements.
- Risk Management: Balance individual choice and independence with effective risk assessment and management.
- Record Keeping: Maintain accurate, detailed, and up-to-date case records and reports.
- Professional Practice: Work in accordance with Social Work England standards and Somerset Council policies and procedures.
