What you will do:

• Undertake comprehensive needs assessments, reviews, and reassessments in accordance with the Care Act 2014.
• Develop, implement and monitor person-centred care and support plans, ensuring outcomes are achieved efficiently.
• Manage a caseload of adults with diverse and complex needs, prioritising workload effectively to deliver the best outcomes.
• Lead and contribute to safeguarding enquiries under Section 42, ensuring risk is appropriately managed to protect individuals.
• Work within a multi-disciplinary team, liaising seamlessly with NHS partners, housing services, providers, and various voluntary sector organisations.

Person Specification:

Qualifications:

• Degree in Social Work or an equivalent recognised qualification.

Registration & Compliance:

• Must be registered with Social Work England (SWE).
• Ability to apply key legislation, including the Care Act 2014, Mental Capacity Act 2005, Equality Act 2010, and Safeguarding Adults procedures effectively.
