What You Will Do

Conduct strengths-based assessments in accordance with the Care Act 2014 to identify individual needs and outcomes.

Develop and manage personalised care and support plans that promote independence and wellbeing.

Undertake Mental Capacity Assessments and facilitate best-interest decisions in line with the Mental Capacity Act 2005.

Support service users and carers through complex decision-making processes.
