• Work within a proactive Transformation Team focused on enhancing adult social care services.

• Complete Care Act assessments and ensure timely, high-quality outcomes.

• Manage and review care packages across Learning Disabilities and Mental Health services.

• Use Liquid Logic to maintain accurate, up-to-date case records and documentation.

• Attend team meetings and contribute to service improvement strategies.

• Collaborate with multi-disciplinary professionals to deliver integrated, person-centred support.

• Contribute to innovation and continuous improvement across service delivery.
