About the Role

An exciting opportunity has arisen for an Adult Social Worker - Locality Team in Bromley, offering £32 per hour in an ongoing assignment. This full-time role (36 hours per week) combines three days in the office with flexible hybrid working arrangements.

You will play a key role in supporting the communities of Penge and Beckenham over an initial three-month period, with the potential for extension. This is a fantastic opportunity to make a meaningful difference while enjoying a flexible and supportive working environment.

Key Responsibilities

• Assessment & Care Planning:

  • Conduct comprehensive assessments and develop personalised, strengths-based support plans.

• Multi-Disciplinary Collaboration:

  • Work closely with health professionals, community services, and partner agencies to provide holistic care.

• Safeguarding & Wellbeing:

  • Support safeguarding processes for adults at risk, promoting independence and wellbeing in line with statutory responsibilities.

• Case Management:

  • Attend and actively contribute to case discussions, reviews, and strategic planning meetings.

• Access to Services:

  • Facilitate access to appropriate services, community resources, and support networks.

• Record Keeping & Reporting:

  • Maintain accurate case records and prepare professional reports to inform care decisions and outcomes.
