What You Will Do:

• Conduct comprehensive assessments to identify the needs of adults
• Develop and coordinate tailored care and support plans
• Work collaboratively with external agencies and community services
• Maintain accurate and compliant case records and reports
• Provide emotional support and guidance to service users
• Lead and participate in safeguarding investigations to protect vulnerable adults

Requirements:

• Qualified Social Worker with Social Work England registration
• Experience within adult social care settings
• Strong assessment, safeguarding, and care planning skills
• Excellent communication and organisational abilities
