What you will do:

• Conduct assessments, reviews, and reassessments under the Care Act 2014 to develop personalised care plans.

• Respond to safeguarding concerns, ensuring risk assessments and protective measures are in place.

• Perform Mental Capacity Assessments and contribute to decisions that uphold individuals' rights.

• Manage a caseload, reviewing services and performing assessments to ensure care effectiveness.

• Collaborate with health professionals, housing services, and other agencies to coordinate comprehensive care plans.

• Maintain detailed, accurate case records in line with professional standards and agency policies.

• Engage in ongoing supervision and training to foster professional growth.

Person Specification:

• Degree in Social Work (BA / MA / DipSW).

• Registration with Social Work England (SWE).

• Experience within Adult Social Care in a local authority or statutory setting, particularly in Care Act assessments and safeguarding.
