We have an exciting opportunity for an experienced Senior Social Worker to join the Mental Health Hospital Discharge Team in Ealing. What you will do
- Carry out strengths-based assessments and reviews for adults with primary mental health needs in line with the Care Act, developing person-centred support plans.
- Complete Mental Capacity Act assessments and prepare Social Circumstances Tribunal reports where required.
- Act as a Social Supervisor for conditionally discharged Section 37/41 patients, ensuring statutory responsibilities are fulfilled.
- Lead safeguarding enquiries as a Safeguarding Enquiry Officer, working alongside the Safeguarding Adults Manager to manage risk and achieve positive safeguarding outcomes.
- Participate in the team duty rota, screening referrals, responding to safeguarding concerns, and providing advice to service users, carers, and partner professionals.
- Work collaboratively with West London NHS Trust and multidisciplinary colleagues to deliver effective hospital discharge planning and coordinated community support.
Requirements
- Qualified Social Worker with current Social Work England registration.
- Significant experience within adult mental health social work, including hospital discharge.
- Sound knowledge of the Care Act, Mental Capacity Act, safeguarding legislation, and relevant mental health legislation.
- Strong assessment, risk management, and multidisciplinary working skills.
- Ability to manage complex cases while delivering high-quality, person-centred interventions.
