What you will do:

- Conduct comprehensive triage and assessments within the Children’s Triage Team to identify needs and risks
- Collaborate with multi-disciplinary teams to formulate and implement effective care plans
- Liaise with families, carers, and other professionals to ensure the delivery of coordinated services
- Maintain accurate and timely records in accordance with agency and statutory requirements
- Provide expert advice and guidance to individuals and families, helping them navigate social care systems
