What you will be doing You will join an inclusive, supportive team that works well together to provide care to people with learning disabilities and mental health conditions. Working alongside a multidisciplinary team, you will use your excellent communication skills and experience as a Support Worker with a background in Learning Disabilities &amp; Autism to provide care and support people through their care pathway. You'll work 37.5 hours per week, working 7.5 hours a day with shift patterns of 7am-2.30pm and 2.30pm-10pm, with weekends on a rota basis. Extra hours can be discussed if required. Where you will be working Location: 78 Park Road, Hampton Hill Hampton, Middlesex TW12 1HR 78 Park Road is a seven bed, mixed gender community based home. People supported at 78 Park Road have severe learning disabilities and associated health needs, autism and behaviours which, due to their diagnosis may be seen as challenging. People may come to 78 Park Road as part of their care pathway, either moving from their community placement or the family home because they require more structured care and support. The service is staffed 24 hours a day, 7 days a week, by a dedicated team made up of the Registered Manager and Support Workers. All residents are registered with the local GP which allows access to specialist clinicians.
