Social Care in Kensington and Chelsea is offering an exciting opportunity for a Social Worker within the Mental Health Review Placement Team, earning £38.00 per hour. Available on an ongoing basis, this position welcomes both full-time and part-time applicants and promises a dynamic work environment where every day brings new challenges and rewards.
Join our team where travel is part of the job, including visits to out-of-borough placements that could be situated anywhere. Do not worry about travel expenses; train tickets will be provided for reviews outside of the borough.
What you will do:
- Reviewing out-of-borough placements
- Conducting Care Act assessments
- Managing move-on and step-down processes
- Undertaking Mental Capacity Act and Best Interest decisions
- Ensuring representation and advocacy for clients
- Raising safeguarding alerts
- Participating in inquiries
- Attending both team and inter-agency meetings
- Working closely with providers to maintain current placements and find new ones
- Ensuring that placements are suitable for individuals' needs and the correct borough is identified for aftercare fees under section 117
