What you will do:
- Undertake comprehensive strengths-based assessments under the Care Act 2014, including risk assessments and support planning.
- Manage a varied caseload, ensuring timely reviews, interventions, and outcome-focused care plans.
- Lead on safeguarding enquiries in line with Section 42 responsibilities and council procedures.
- Work collaboratively with NHS partners, housing, voluntary sector agencies, and internal teams to coordinate holistic support.
- Promote independence, reablement, and community-based solutions to reduce long-term dependency on services.
Qualifications:
- Degree in Social Work or equivalent recognised qualification.
- Registered with Social Work England.
Registration and Compliance:
- Current Social Work England registration.
- Strong working knowledge of the Care Act 2014, Mental Capacity Act 2005, and Equality Act 2010.
- Understanding of safeguarding frameworks and Making Safeguarding Personal principles.
