What you will do:
- Work closely in a multi-disciplinary team to deliver comprehensive community mental health services
- Implement the Care and Treatment Planning approach, ensuring effective community-based health and social care to enhance service users' independence and dignity
- Fulfil the duties of the Approved Mental Health Professional (AMHP) when needed, acting swiftly in response to service user requirements
- Provide guidance and support to less experienced staff, fostering their growth and development
- Conduct thorough assessments to identify individual needs and coordinate planned care and support for service users and their carers
- Evaluate risk levels and organise services to manage and mitigate risks effectively
- Deliver a social work service aligned with relevant legislation, regulations, and guidance for mental health service provision
