Duties:

<ul>
<li>Attending regular MDT meetings and liaising with colleagues from other services (both internally and externally such as CRHTT, Adult Social Care and third sector organisations)</li>

<li>Supporting and managing a caseload undertaking face to face visits in the community setting including home visits</li>

<li>Working with service users and their families/carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal/external stakeholders</li>

<li>Undertaking urgent/routine initial assessments on behalf of the team</li>

<li>Working alongside Lead Care Professionals and supporting those currently under the Care Programme Approach</li>

<li>Participating in the duty worker rota (Monday to Friday 09:00am to 17:00pm)</li>

<li>Medication administration (if qualified to do so) with opportunities to enrol in local university training programmes in medication management</li>
</ul>

Requirements of the Social Worker:

<ul>
<li>Degree or equivalent in social work</li>

<li>Current Social Work England registration</li>

<li>Experience of supporting people and their families, preferably in the community</li>

<li>Demonstrate a strong understanding of your role as a Care Coordinator/Assessor and join our dedicated multidisciplinary team working closely with a range of partners to provide appropriate community interventions for those individuals referred to the team</li>

<li>Able to demonstrate an appropriate level of knowledge of mental health practice from previous experience</li>

<li>Demonstrate effective risk assessment and risk management experience</li>

<li>Experience in the delivery of clinical interventions</li>

<li>Experience in working in team delivering person centred treatment packages</li>

<li>Knowledge of medication and the monitoring of its effects</li>

<li>Knowledge of psychosocial interventions</li>

<li>An understanding of the policies and guidelines both local and national e.g., National Institute for Clinical Excellence guidelines (Government initiative)</li>
