What you will do

  • Case Management: Manage a varied caseload of adults with complex mental health needs.
  • Assessment: Complete and review Care Act assessments, ensuring person-centred and outcome-focused support planning.
  • Mental Capacity: Undertake Mental Capacity Act assessments and make Best Interest decisions where appropriate.
  • Safeguarding: Lead and contribute to safeguarding enquiries under Section 42 of the Care Act.
  • Multi-Agency Working: Collaborate with NHS mental health services, community teams, and partner agencies to deliver integrated support.
  • Care Planning: Develop, implement, and review effective care and support plans.
  • Strengths-Based Practice: Promote independence, recovery, and resilience through a strengths-based approach.
  • Professional Reporting: Prepare comprehensive assessments, reports, and court documentation where required.
  • Record Keeping: Maintain accurate, timely, and compliant case records.
  • Statutory Responsibilities: Deliver services in accordance with legislation, council policies, and professional standards.
