What you will do
- Discharge Coordination: Facilitate safe and efficient hospital discharges in collaboration with health partners.
- Assessment: Conduct holistic, strengths-based assessments to identify care and support needs.
- Care Planning: Develop, implement, and review person-centred care plans.
- Multi-Agency Working: Work closely with healthcare professionals and community services to ensure integrated care.
- Advocacy: Promote patient-centred approaches, ensuring individuals' rights and preferences are respected.
- Case Recording: Maintain accurate and timely records using systems such as Mosaic.
Qualifications / Requirements
- Qualification: Degree in Social Work or equivalent.
- Registration: Registered with Social Work England.
- Experience: Recent experience within hospital teams or Transfer of Care settings.
- Skills: Strong assessment, communication, and decision-making abilities.
- IT Knowledge: Familiarity with Mosaic system is desirable.
